CASE INVESTIGATION FORM FOR ARRIVING PASSENGERS AT NAIA

The Embassy wishes to inform the public that effective immediately, all arriving passengers in Manila will be required to fill up a Case Investigation Form which will be collected upon their arrival at the Ninoy Aquino International Airport (NAIA).

For their own convenience, passengers travelling to the Philippines are encouraged to download the form, print and fill it up before their arrival in Manila.
