AUTHENTICATION, NOTARIAL AND CIVIL REGISTRATION SERVICES

1.    Authentication of documents

Effective 14 May 2019, the Apostille Convention has entered into force in the Philippines. The Apostille has replaced the authentication ("red ribboning") process previously used by the Philippine Government for the legalization of documents.

Any document issued in the Philippines for use overseas must be apostillized by the Department of Foreign Affairs.  The Apostille replaces the Certificate of Authentication ("red ribbon") as proof of authentication.  For more information on apostillization in the Philippines, please visit the DFA Q&A on the new Apostille.

Any document issued in Poland, Estonia, Latvia, Lithuania or Ukraine for use in the Philippines does not need authentication anymore by the Philippine Embassy if the document has been duly apostillized by the respective Ministry of Foreign Affairs of these countries.

However, the requirement to have documents translated into English (if the original document is in Polish, Estonian, Latvian, Lithuanian or Ukrainian) remains, and the English translation needs to be submitted together with the original document when presented to the appropriate Ministry of Foreign Affairs for the Apostille.

Red Satin Ribbon Attachment

Effective 15 June 2019, the Embassy will no longer affix red satin ribbons on its notarized documents pursuant to instructions from the Department of Foreign Affairs.

2.    Notarial services

Requirements for different types of notarial services:

a.    Administration of oaths/affirmations/affidavits:

  • Form (available at the Consular Section)
  • Copy of passport
  • Payment of notarial fee: PLN 100

b.    Acknowledgements

  • Document to be acknowledged (please sign the document in the presence of the consular officer)
  • Copy of passport
  • Payment of acknowledgement fee: PLN 100

Civil registration services

 The marriages and births of Filipino nationals within the Embassy’s jurisdiction should be reported to the Embassy within 12 months from the date of the event. Requirements are as follows:

 Report of Marriage (Download Form)

  • Four duly-accomplished report of marriage forms
  • Four copies of the original marriage certificate with English translation apostillized by the Foreign Ministry
  • Four copies of the applicants’ passports
  • Four passport-size colored photos of the applicants
  • Affidavit of delayed registration if the report was made more than twelve months after the marriage took place (a fee of PLN 100 will apply)
  • Report fee of PLN 100

Report of Birth (Download Form)

  • Four duly-accomplished report of birth forms
  • Four copies of the original birth certificate with English translation apostillized by the Foreign Ministry
  • Four copies of the parents’ Report of Marriage or DFA-apostilled marriage certificate issued by the Philippine Statistics Authority
  • Four copies of the parents’ passports
  • Affidavit of delayed registration if the report was made more than twelve months after the marriage took place (a fee of PLN 100 will apply)
  • Report fee of PLN 100

Report of Death (Download Form)

Please contact the Consular Section at 22 490 2025 or the Duty Officer at 694 736 488 (after office hours) to report the death of a Filipino national.

4. Dual citizenship under R.A. 9225

Applicable to natural-born Filipino nationals who have acquired Polish, Estonian, Latvian Lithuanian citizenship. Requirements are as follows:

  • Proof of natural-born Philippine citizenship – Original plus four copies of PSA birth certificate apostilled by the DFA
  • Proof of acquisition of foreign citizenship – Original plus four copies of naturalization certificate with English translation verified by the Foreign Ministry
  • Two accomplished petition forms for reacquisition of PH citizenship

Additional requirements:

  • Original and four copies of DFA-apostilled marriage certificate or Report of marriage, for married applicants
  • Most recent Philippine passport issued to applicant plus four copies of the data page
  • Foreign passport and identification card plus four copies
  • Recent passport-size colored photo – four pieces
  • For minor children born after the applicant received foreign citizenship, original birth certificate with verified English translation (report of birth must be submitted)
  • Payment of fees: PLN 200 for principal applicant, PLN 100 for every minor child included in the petition

Note: The consular officer may require additional documents if there is a discrepancy between the applicant’s name in his/her birth certificate and in his/her naturalization certificate/foreign passport.

5. Certificate of Legal Capacity to Marry / Legal Capacity to Contract Marriage (LCCM) Certificate

  • Application form, duly filled up
  • Recent passport-size colored photos – one each for applicant and prospective spouse
  • Original plus one copy of applicant’s DFA-apostilled Birth Certificate issued by the Philippine Statistics Authority (PSA)
  • Original plus one copy of applicant’s DFA-apostilled Certificate of No Marriage record (CENOMAR) issued by the PSA
  • Original plus one copy of the DFA-authenticated Affidavit of Parental Consent issued by applicant’s parents (if applicant is from 18 to 21 years of age)
  • Original plus one copy of the DFA-apostilled Affidavit of Parental Advice issued by applicant’s parents (if applicant is from 22 to 25 years of age)
  • Original plus one copy of prospective spouse’s Birth Certificate (with English translation, if Polish document is not readable in English  / has no English subtitles) 
  • Original plus one photocopy of Certificate of Marital Status of prospective spouse, with English translation
  • Original plus copy of the passport - one each for the applicant and prospective spouse
  • Payment of PLN 100,- notarial fee for the application and PLN 100,- for the LCCM certificate
  • Other relevant documents as may be determined by the consular officer

Note: A  Notice for Marriage must be posted for at least ten (10) working days before an LCCM certificate can be issued.
