AUTHENTICATION, NOTARIAL AND OTHER SERVICES

APOSTILLE AND AUTHENTICATION OF DOCUMENTS

Since 14 May 2019, the Apostille has replaced the authentication ("red ribboning") process previously used by the Philippine Government for the legalization of documents.

Any document issued in the Philippines for use overseas must be apostilled by the Department of Foreign Affairs.  For more information, please visit the DFA Q&A on the new Apostille.

NOTARIAL SERVICES

Notarial documents include the administration of Oaths, Affirmations, various Affidavits including Affidavit of Support and Guarantee, Acknowledgements, Certifications and the likes. Some notarial forms are available for download here. Notarial forms may also be supplied.

How to Apply

Appointment is needed for notarial services. You may apply for an appointment here.

Requirements

  • Duly Accomplished Form (available at the Consular Section or download here. Please note that documents should be signed in the presence of the Consular Officer).
  • Copy of valid passport and/or National ID.

Fees

  • The payment of Notarial Fee is preferably in cash: PLN 125
  • Expedite Processing Fee: PLN 50

NBI CLEARANCE CERTIFICATE

Who can apply?

Filipinos in Poland who last applied before 2014, or have changed their names since their last NBI Clearance (females who got married) or are first time applicants for NBI Clearance.

How to Apply

Accomplish NBI Clearance Fingerprint Card (NBI Form No.5) at the Embassy. The Embassy staff will supervise and assist you in taking your fingerprints.

Requirements

Bring/Submit a recent 2x2 ID photograph with white background taken within the last three (3) months.

Fees

  • The payment of Attestation Fee is preferably in cash: PLN 125
  • Expedite Processing Fee: PLN 50

What to do after the application

Once done, you may send the following requirements to your authorized representative in the Philippines who will file the application on your behalf with the NBI:

  • Authorization Letter
  • Old NBI Clearance Certificate
  • Attested NBI Form No.5 with 2x2 photo
  • Photocopy of valid passport
  • Fee of PHP 200.00 (in the Philippines)

PAYMENT OF CONSULAR FEES BY BANK TRANSFER

Cash payment is preferred.

Bank transfer payments are Nonrefundable. Do not make payments unless you are cleared to pay.

Bank charges shall be on the account of the payor.
