PROCEDURES FOR SECURING PHILIPPINE DRIVER’S LICENSE CERTIFICATION AND HISTORY
The Filipino Community in Poland, Estonia, Lithuania, Latvia, and Ukraine are hereby informed that the Philippine Land Transportation Office (LTO) Citizen’s Charter 2023 (pages 65-67) provides for the procedures that are required to be followed in the Issuance of Driver’s License Certification and History for purposes locally or abroad.
In view of the above LTO procedures, the Philippine Embassy in Warsaw no longer accepts requests for verification of Philippine Driver’s License since 15 May 2023.
The following LTO requirements and procedures shall apply.
Requirements
- One (1) photocopy of the Driver’s License and/or Official Receipt (OR)
- In case of lost Driver's License, original affidavit of loss. If the Driver's License holder is out of the country, one (1) photocopy of valid passport showing the first page, visa page and latest date of arrival and departure.
- If applied by a representative, the representative should present the authorization letter with one (1) photocopy of any valid government/company issued ID with photo and signature of the applicant and representative. (The authorization letter is from the Driver's License holder. Both the Representative and Driver's License holder should present IDs from any issuing government agency/company).
Procedures
- Applicant or representative shall secure the Request Form for the application of license certification and accomplishes the same at the LTO License Section, Central Office and Regional Offices.
- Applicant or representative shall pay a Certification Fee of PHP 100.00.
- Upon receipt of the DL Certification, applicant or representative shall proceed to the DFA for the Apostille of the DL Certification document. Click here for the requirements and procedures for securing the Apostille.
- Once with Apostille, the Certification may be submitted to the local Driver’s License issuing authorities (whether in Poland, Estonia, Latvia, Lithuania and Ukraine).
- The issuance of the Apostille Certification may also be verified by both the applicants and local issuing authorities via the E-Registry for Apostilles issued starting 01 January 2023.
